I601A Form Draft Version from 2012
Below is a copy of the draft version of the I-601A form. Please note that the final version of the I601A form may be different than the draft version and you should not rely on the draft version of the form when preparing your application. The draft version of the form may be helpful in identifying the personal biographic information that you’ll need in order to complete the I601A form when the final version becomes available. The final version of the I601A form is expected to be released on March 4, 2013.

I601A Form Biographic Data Checklist Based on Draft Form
Although the final form may require different information, you may find the following checklist based on the draft form to be helpful:
Information About the Applicant
- A Number
- Social Security Number
- Full Name
- Other names used
- Home address
- Mailing address
- Phone number
- E-mail address
- Date of birth
- City or town of birth
- Province of birth
- Country of birth
- Country of citizenship
Information About Applicant’s Last Entry to the United States
- Date of last entry
- Place of last entry (city/town and state)
- Immigration status at time of last entry
Information About Each of Applicant’s Previous Entries to the United States
- Place of entry (city/town and state)
- Date entered the United States
- Date departed the United States
- Immigration status at time of entry
Note: Your immigration lawyer will also want to know if you spoke with any immigration officers when you entered the United States and if you were ever arrested by immigration officers or placed in deportation proceedings at any time when you have been in the United States.
Immigration or Criminal History Records
- Are you currently in removal proceedings?
- Are you subject to a final order of removal, deportation, or exclusion, or to the reinstatement of a prior removal order?
- Have you ever given false or misleading information to a U.S. Government official while applying for any immigration benefit or to gain entry or admission to the United States?
- Have you ever assisted the entry of someone, even a family member, into the United States without the benefit of a valid travel document?
- Have you ever committed a crime for which you were not arrested?
- Have you ever been arrested, cited, or detained by a law enforcement officer (including immigration and military officers) for any reason other than traffic violations in the United States, your home country, and/or any other country?
- Have you ever been charged, indicted, imprisoned or jailed for any crime or offense?
- Have you ever been convicted of a crime?
- For each arrest, citation, detention, charge, indictment, imprisonment or time in jail, you’ll need all cour dispositions and information about the event (city/town, state, country, date, and description of the event)
Information About Immigrant Visa Petition and Consular Visa Processing
- USCIS receipt number for the approved visa petition
- Petitioner’s full name
- Relationship between the petitioner and the applicant
- Consular case number (NVC case number)
- Have you already been scheduled for an immigrant visa interview?
Information About Qualifying Relative
- Full name
- Relationship to the applicant (US citizen spouse or US citizen parent)
- Name and relationship of any other qualifying relatives
